Job Description:

Trinity Health PACE seeks a dedicated Physical Therapist to join our multidisciplinary team, providing targeted, comprehensive physical therapy services to elderly adults. Our ideal candidate will embody our commitment to enhancing the mobility, independence, and quality of life of our participants. Working at our PACE centers, you will implement care plans finely tuned to the individual needs of our participants, all within a supportive and collaborative environment focused on preventative and rehabilitative care. Evaluate the physical condition of patients to diagnose problems and plan appropriate treatments Develop treatment plans by identifying a problem list and methods to achieve identified goals Implement physical therapy interventions tailored to individual patient needs, including exercises and manual therapy Collaborate with a multidisciplinary team to provide comprehensive care Document patient care and progress, updating treatment plans as necessary Educate patients and their families on treatments, therapy exercises, and strategies for coping with physical limitations Adhere to all professional, ethical, and health and safety guidelines Proficiency in developing and executing treatment plans Strong interpersonal skills for effective communication with patients and families Ability to collaborate effectively with other healthcare professionals Strong organizational and documentation skills Experience with geriatric populations is preferred Medical Insurance Dental Insurance Vision Insurance Valid Physical Therapist license CPR certified
