Requirements Licensure/Certification/Registration: Physician MD/DO
credentialed from the Indiana Medical Licensing Board obtained
prior to hire date or job transfer date required.Advanced Life
Support credentialed from the American Heart Association (AHA)
preferred.Education: Doctor of Medicine (MD) or Doctor of
