Growing together. -The Health and Social Services Manager will lead a care management team while overseeing day-to-day clinical operations and health plan clinical initiatives to support achieving quality and affordability targets. - The HSS Managers will support a team within different regions in Central Indiana. - Responsibilities will include process improvement, execution of clinical decision processes, and team management, to include performance management and implementation of program initiatives. -If you reside in Indiana, you will have the flexibility to work remotely* as you take on some tough challenges. - - -Primary Responsibilities: -

• Selects, manages, develops, mentors and supports staff in designated department or region
• Develops clear goals and objectives for performance management and effectively communicates expectations, and holds the team accountable for results
• In order to meet the unique needs of our members, have an intimate understanding of the contractual requirements
• Identify, select, structure, and prioritize process improvement projects, ultimately implementing changes to meet program requirements
• Conducts monthly audits of staff and participates in all other required audits and business monitoring
• Ensures standardized execution of workflow processes, including conducting performance audits, quality reviews, and compliance adherence
• Participates in training and coaching of direct reports as needed
• Collaborates across Optum and UHG and interacted with Medical Directors, Senior Leaders, Quality, and other stakeholders - -You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. -Required Qualifications: -
  • Active and unrestricted RN license in the state of Indiana or Master's level social worker -
  • 5+ years of experience in providing care coordination to older adults
  • 4+ years of experience working within the community health setting in a health care role
  • 3+ years management experience
  • Experience in LTSS or working with the program population
  • Experience facilitating and working in coordination with an Interdisciplinary Team
  • Experience coaching or mentoring staff
  • Intermediate level of experience with Microsoft Word and Excel
  • Proven ability to navigate a TEAMs environment
  • Resident of IndianaPreferred Qualifications:
    • Case Management experience
    • Experience with LTSS or Waivers
