Job Description:
Overview:SUPERVISION: This position reports to the Treatment
Director or the Clinical Director, who in turn reports to the
ExecutiveDirector.POSITION PHILOSOPHY: Functions as a team member
with clinical expertise. Is knowledgeable in assessment,
behavioralmanagement, and treatment modalities. Is primarily
responsible for developing, coordinating, and monitoring the
support of studentson their assigned case load to be sure that each
student receives a comprehensive, person-centered treatment program
of support. TheCase Manager is the central communicator and
collaborates with internal and external stakeholders in the design
andimplementation of the treatment program and assures that each
individual treatment program is consistently implemented
anddocumented.Responsibilities:ESSENTIAL FUNCTIONS LISTED BUT NOT
LIMITED TO:I. WORKS A SCHEDULE THAT MEETS THE NEEDS OF THE
DEPARTMENT AND THEAGENCY1. Works scheduled hours as designated by
supervisor.2. Submits requests for the use of benefit time and
schedule changes to supervisor.3. Uses timekeeping system per
agency procedures.4. Accepts caseload and team assignments per
supervisor.5. Establishes daily work schedule in coordination with
others' schedules and informs supervisor.II. MEETS THE OVERALL
AGENCY & DEPARTMENTAL EXPECTATIONS:1. Functions within legal,
licensing, agency, and professional standards in maintaining a
therapeutic environment for the students.2. Maintains a valid
Indiana driver's license and excellent driving record to utilize
agency vehicles and/or transport students in personal vehicle.
Maintains personal auto insurance with current insurance
information on file with Human Resources. Informs supervisor
regarding changes in license and/or personal auto insurance.3.
Demonstrates knowledge and appropriate implementation of company
policies, procedures, and Employee Handbook information.4. Observes
rules of confidentiality, both on the job and in the community.5.
Ensures the proper use of agency forms per policies and
procedures.6. Maintains a positive, open, and supportive working
relationship between departments in all actions and
communication.7. Conducts self in a positive, respectful and
collaborative manner. Demonstrates proactive actions and decision
making that fosters supportive work relationships and promotes a
healthy and safe environment. Communicates effectively verbally and
in writing with internal and externally, maintaining professional
conduct and confidentiality.III. IMPLEMENTATION:Individual and Unit
Programming1. Prepares, develops, and reviews the student's overall
treatment and support plan. Completes assessments of the student,
including the Life Skills Assessment (LSA) and Ansell Casey.
Completes monthly reports and progresses monthly on students'
behavioral and life skill IEP goals.2. Ensures that programming is
developed that is of a therapeutic nature and takes into
consideration age, level of functioning, likes/dislikes,
cultural/ethnic differences, resident risks, appropriateness of
activity, and recreation budget.3. Establishes Life Skill goals and
support strategies based on input from internal and external team
members including therapists, program coordinators, direct support
staff, and families. Monitors the implementation of treatment goals
and strategies.4. Monitors implementation of treatment strategies
per the Individual Education Program (IEP)/Individual Treatment
Plan (ITP) by direct contact, consultation, and monitoring of
required paperwork.Qualifications:QUALIFICATIONS: Minimum of 1-year
case management/QSP experience. Bachelor's degree from an
accreditedcollege/university in a human services field or
equivalent combination of education and case management/QSP
experience. Minimumof one year of demonstrated knowledge and skills
in working with and supporting children, adolescents and/or young
adults diagnosedwith emotional, mental health, neurological and/or
developmental disabilities.
